Here is how to fill the DITO SIM registration form as well as how to undergo the verification process in order to register your SIM card in 2024.

DITO SIM Online Registration Procedures

Step 1: Open DITO Registration Link Or Mobile App

• New DITO subscribers can register their SIMs through the SMS or link at https://register.dito.ph 
• While existing subscribers can register via the DITO mobile app which can be downloaded at Google Play or the App Store .
• On the respective page tap the “Register Your SIM Card Now” button

Step 2: Fill out the DITO SIM registration form

• The next step is to fill out the SIM registration form with your personal information and upload your ID card to verify your phone number as follows:
• Identity Type:
  • Filipino: Filipino citizens of legal age.
  • Filipino-Dependent: When registering on behalf of a minor or dependent as a parent or guardian.
  • Juridical Entity: If you represent a corporation, non-government organization and is authorized to register a SIM on its behalf.
  • Foreigner with Tourist Visa: For foreign nationals with Tourist Visa
  • Foreigner with Other Visa: For foreign nationals with other visas other than Tourist Visa
• Mobile Number: Enter your 11-digit DITO number. Note that the first two digits are already provided, so just type the last 9 digits.
• First Name: State your first name
• Middle Name: State your middle name.
• Surname: State your surname.
• Date of Birth: State your your date of birth as follows: MM/DD/YYYY
• Gender: State your gender
• Government ID: Choose from a list of government-issued IDs.
• ID Number: Enter the ID card number displayed at the front of the ID
• House Number: State your house number
• Street: State your street
• Province, City and Barangay: Select your province, city and barangay
• ZIP Code: Provide your ZIP code or postal code.
• Identification Card: Upload a photo of your valid government-issued ID. This should not exceed file 5MB.
• User Photo: Upload a selfie of you holding your ID card. This should not exceed file 5MB.
• Mobile Number Verification: Ta[p the “Get OTP” button to be sent a one-time password (OTP) code then enter the code to get verified
• Data Privacy Statement: Tick the checkbox to indicate that you agree to the Data Privacy Statement.

Step 3: Submit

• Finally “Submit” the DITO SIM registration form to complete the registration process

DITO Sim Registration Requirements

Requirements for Individuals

• a valid government-issued ID card
• Full name
• Sex
• Present or official address
• Type of ID presented
• Date of birth
• ID number
• Mobile number with serial number

Requirements For Minors

• Any valid government-issued ID; and
• Consent of the minor’s parent or legal guardian to register the SIM.

Requirements for Foreign Nationals

Foreign nationals residing in the Philippines and tourists visiting the country will need to submit the following details when registering their SIMs:

• Date of birth
• Passport
• Address in the Philippines
• Full name
• Nationality
• ID number or number of document presented
• Mobile number with serial number

Requirement for Tourists

• A passport with the bio page or pages displaying the temporary visitor’s visa stamp or details, Evidence of residence in the Philippines, like a hotel booking or accommodation reservation, or alternatively, an affidavit or letter from the owner of the residence where the tourist is staying, A return ticket to the tourist’s home country, or any ticket indicating the departure date and time from the Philippines.

Requirements for Foreign Nationals With Other Visas

• An Alien Certificate of Registration (ACR) ID card provided by the Bureau of Immigration or an official identification issued by a visa-issuing agency
• An Alien Employment Permit granted by the Department of Labor and Employment (DOLE)
• Student registration and identification for students
• For Persons of Concern, a travel or admission document issued by the Department of Justice.

Requirements for Businesses

• Full name of authorized signatory
• Certificate of Registration
• Business name
• Business address
• Duly adopted resolution designating the authorized representative (for corporations)
• Special Power of Attorney (for other juridical entities)
